The 2nd edition of this book describes the recent techniques, strategies, and drugs that have been demonstrated by multicenter randomized trials to influence survival in critically ill, defined as those who have acute failure of at least one organ, due to either a pathological condition or a medical intervention, and require intensive care treatment. The scope is accordingly wide, with coverage of topics as diverse as noninvasive mechanical ventilation, protective ventilation, prone positioning, intravenous salbutamol in ARDS, high-frequency oscillatory ventilation, mild hypothermia after cardiac arrest, daily interruption of sedatives, tranexamic acid, diaspirin cross-linked hemoglobin, albumin, growth hormone, glutamine supplementation, tight glucose control, supranormal oxygen delivery, and hydroxyethyl starch in sepsis. The topics selection was performed with the help of hundreds of specialists from dozens of countries; they expressed via web if they agreed or not with these topics and if they used them in their daily clinical practice. This book explores the ethical dilemma clinicians may face when disclosing a diagnosis of atypical sex. The moment of disclosure reveals an epistemic incompatibility between scientific fact and social meaning in relation to sex. Attempting to assess the bio-psychosocial implications of this dilemma highlights a complex historic antagonism between fact and meaning making satisfactory resolution of this dilemma difficult. Drawing on David Hume, WVO Quine and Michel Foucault the author presents an integrative model, which views scientific fact and social meaning as codetermining threads in one fabric of knowledge. From this epistemic perspective, the ethical dilemma is understood as a tear in the fabric signifying a rupturing of ontological integrity. To mend this tear and resolve the ethical dilemma three metaphysical perspectives are considered: essentialism, naturalism and emergentism. The book''s unique features include: an exploration of the impact of diagnostic disclosure on people with atypical sex (intersex); a synthesis of the epistemic perspectives of social and natural science facilitating interdisciplinary collaboration; a critical evaluation of three metaphysical perspectives on atypical sex (intersex); the application of Hume''s epistemological and moral distinctions to contemporary biomedicine and bioethics. This book introduces ecologists to the wonderful world of modern tools for data analysis, especially multivariate analysis. For biologists with relatively little prior knowledge of statistics, it introduces a modern, advanced approach to data analysis in an intuitive and accessible way. The book begins by reviewing some core principles in statistics, and relates common methods to the linear model, a general framework for modeling data where the response is continuous. This is then extended to discrete data using generalized linear models, to designs with multiple sampling levels via mixed models, and to situations where there are multiple response variables via model-based approaches to multivariate analysis. Along the way there is an introduction to: important principles in model selection; adaptations of the model to handle non-linearity and cyclical variables; dependence due to structured correlation in time, space or phylogeny; and design-based techniques for inference that can relax some of the modelling assumptions. It concludes with a range of advanced topics in model-based multivariate analysis relevant to the modern ecologist, including fourth corner, latent variable and copula models. Examples span a variety of applications including environmental monitoring, species distribution modeling, global-scale surveys of plant traits, and small field experiments on biological controls. Math Boxes throughout the book explain some of the core ideas mathematically for readers who want to delve deeper, and R code is used throughout. Accompanying code, data, and solutions to exercises can be found in the ecostats R package on CRAN.
